Documentation to be submitted for assessment by applicants from the European Union

Applicants must submit the following documents for assessment:

a) Proof of nationality (Identity Document, passport);
b)
Curriculum vitae;
c)
Training documents [course certificate/diploma, transcript of records (by years or semesters), including internships and practical training forming part of the course, noting the specific and overall ECTS) and if not included in the above , documentation confirming start and completion dates of the course and, where appropriate, subjects completed elsewhere);
d)
Proof of professional activity (type and duration), including internships;
e)
Proof of the type and duration of the professional activity, issued by the competent authority of the Member State of origin (certification);
f)
Proof of training (post-graduate);
g)
Proof of membership of professional associations and category of membership;
h)
Proof of the absence of disciplinary sanctions or declaration to that effect issued by the applicant;
i)
Other documentation to be considered in the application.

Where it is not possible for the applicant to provide all documentation the applicant should include a statement explaining the reasons for the omission.

The local secretariat will notify the applicant of receipt of the documentation submitted within 30 days and, if appropriate, request the missing documents.
