Declaração a preencher pelo Prestador de Serviços
(Declaration to be fulfilled by the Service's Provider)

O registo do prestador de serviços, que vem exercer engenharia em rnPortugal, deverá ser efetuado através do preenchimento da respetiva rndeclaração, juntando os documentos abaixo mencionados (The service’s rnprovider registry to work in the engineering field in Portugal implies rnthe fulfilment of the respective declaration, joining the following documentation):

1 - Cópia do comprovativo da nacionalidade (Copy of document comprovative of nationality);
2rn - Certificado que atesta que se encontra legalmente estabelecido num rnEstado membro para o exercício da profissão e de que não está proibido rnde a exercer (Proof of legal establishment in a home Member State rnallowing the practice of the profession);
3 - Cópia (copy) do título profissional (Proof of Professional titles);
4 - Cópia (copy) Curriculum Vitae.

Toda a documentação deverá ser enviada para o Gabinete de Admissão e rnQualificação através da seguinte morada (The documentation must be sent rnto the Department of Admission and Qualification, to the following rnaddress):
